We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to provide high-quality patient care in a clinical and long-term care environment. The ideal candidate will possess strong clinical skills, excellent communication abilities, and a commitment to delivering exceptional patient-centered care.


Essential Responsibilities

Patient Care

  • Administer medications and treatments as prescribed by healthcare providers.
  • Monitor, assess, and document patient conditions, reporting changes as appropriate.
  • Obtain and record vital signs, patient histories, and clinical observations.
  • Assist physicians, registered nurses, and other healthcare professionals with examinations and procedures.
  • Collect and process specimens for laboratory testing.

Clinical Documentation

  • Maintain accurate and timely patient records in electronic medical record systems.
  • Document assessments, treatments, medications, and patient responses.
  • Complete required reports and clinical documentation in accordance with organizational policies and regulatory standards.

Care Coordination

  • Collaborate with interdisciplinary healthcare teams to support patient care plans.
  • Communicate effectively with patients, families, and healthcare team members.
  • Participate in care planning and patient education initiatives.

Safety & Compliance

  • Follow infection prevention and control procedures.
  • Adhere to all nursing standards, policies, and regulatory requirements.
  • Promote a safe environment for patients, visitors, and staff.
  • Participate in quality improvement and patient safety initiatives.

Qualifications

Required

  • Active and unrestricted Licensed Practical Nurse (LPN) license.
  • Graduate of an accredited practical nursing program.
  • Strong clinical assessment and patient care skills.
  • Excellent communication and interpersonal abilities.
  • Ability to work independently and collaboratively within a team environment.

Preferred

  • Previous experience in long-term care, rehabilitation, outpatient, or acute care settings.
  • Experience with electronic medical record systems.
  • Current CPR/BLS certification.

Salary is dependent on experience : Range is $ 72,000.00 to 94,000.00
