Essential Responsibilities
Patient Care
- Administer medications and treatments as prescribed by healthcare providers.
- Monitor, assess, and document patient conditions, reporting changes as appropriate.
- Obtain and record vital signs, patient histories, and clinical observations.
- Assist physicians, registered nurses, and other healthcare professionals with examinations and procedures.
- Collect and process specimens for laboratory testing.
Clinical Documentation
- Maintain accurate and timely patient records in electronic medical record systems.
- Document assessments, treatments, medications, and patient responses.
- Complete required reports and clinical documentation in accordance with organizational policies and regulatory standards.
Care Coordination
- Collaborate with interdisciplinary healthcare teams to support patient care plans.
- Communicate effectively with patients, families, and healthcare team members.
- Participate in care planning and patient education initiatives.
Safety & Compliance
- Follow infection prevention and control procedures.
- Adhere to all nursing standards, policies, and regulatory requirements.
- Promote a safe environment for patients, visitors, and staff.
- Participate in quality improvement and patient safety initiatives.
Qualifications
Required
- Active and unrestricted Licensed Practical Nurse (LPN) license.
- Graduate of an accredited practical nursing program.
- Strong clinical assessment and patient care skills.
- Excellent communication and interpersonal abilities.
- Ability to work independently and collaboratively within a team environment.
Preferred
- Previous experience in long-term care, rehabilitation, outpatient, or acute care settings.
- Experience with electronic medical record systems.
- Current CPR/BLS certification.
