We are seeking an experienced and compassionate Long-Term Care Registered Nurse (RN) to provide high-quality nursing care across multiple long-term care units. The RN will deliver individualized, patient-centered care focused on promoting comfort, wellness, and quality of life for residents requiring long-term, short-term rehabilitation, hospice, and palliative care services.

This role requires strong clinical judgment, excellent communication skills, and the ability to work collaboratively within an interdisciplinary healthcare team.

Unit Overview

The long-term care division includes:

  • Two 45-bed units serving long-term and short-stay residents
  • One 10-bed hospice and palliative care unit

Schedule

  • Full-time
  • Evening shift: 3:30 PM – 12:00 AM
  • Weekday and weekend rotation required
  • Holiday coverage as needed

Responsibilities

  • Assess, plan, implement, and evaluate individualized nursing care plans
  • Provide direct patient care and supervise support staff as appropriate
  • Administer medications and treatments following established protocols
  • Perform IV therapy, tube feeding, blood transfusions, and other clinical procedures
  • Monitor and document patient status including vital signs, hydration, weight, and behavioral changes
  • Assist residents with activities of daily living (ADLs)
  • Complete admissions, discharges, and patient transfers
  • Educate patients and families regarding care plans and treatment
  • Collaborate with interdisciplinary teams to coordinate continuity of care
  • Maintain accurate electronic medical record documentation
  • Respond appropriately to emergency situations and changes in patient condition

Required Qualifications

  • Active, unrestricted RN license in any U.S. state or territory
  • Minimum of 3 years of current nursing experience
  • Prior long-term care experience required
  • BLS certification through the American Heart Association (AHA)

Education

  • Associate Degree in Nursing (ASN) required
  • Bachelor of Science in Nursing (BSN) preferred
  • Nursing program must be accredited by ACEN or CCNE

Preferred Skills

  • Strong long-term care and geriatric nursing background
  • Experience with hospice and palliative care
  • Excellent communication and teamwork abilities
  • Proficiency with electronic medical records (EMR)
